A sore throat and Pharyngitis are often used interchangeably when a throat infection is concerned. A sore throat is also known as Pharyngitis. Various micro-organisms such as Viruses, bacteria, and fungi may cause a sore throat. Strep throat is also identified as (Group A Beta Hemolytic Streptococcus-GABHS) which can also cause a sore throat. Antibiotics are chosen based only on the most invading micro-organisms, and the severity of the symptoms. Antibiotics are only effective against pharyngitis caused by a bacterial pathogen. If symptoms are severe, testing and broad spectrum antibiotics may be specified. Cost is always considered when choosing antibiotics for the bacterial throat infection.
There are several sore throats inducing pathogens but majorly caused by a virus. Bacterial pathogens do report about 10% of the invading organisms with a strong Gram – positive organism. A weak Gram -negative pathogen may also be the cause of pharyngitis. A small number of the most common bacterial organisms that cause pharyngitis include M. catt, H. influ, and Strep. Signs and symptoms of pharyngitis include:
- A painful sore throat
- Difficulty in swallowing
- Bloody saliva
- Redness of the tonsil and pharynx
- Prolonged hoarseness of the voice, which sometimes last more than a week
- Congestion, cough and possibly fever over 101 degrees Fahrenheit
- Lumps may be found in the neck
When a sore throat reaches 4 to 6 days, and if it is not a result of a more complicated disease, such an individual should look for medical treatment as soon as possible.
Many argue the case of diagnosing before prescribing any antibiotics for an uncomplicated sore throat due to the high occurrence viral infection. There is no specific time frame to start the medicine, but some physicians report that if a sore throat continues for 72 hours antibiotic therapy should be started because there is every possibility that the pathogen may be bacterial.
If the symptoms of a sore throat are minimal, a high dose Penicillin or First Generation Macrolide Antibiotic is necessary and cost – effective to treat the infection.
If the symptoms are systemic such as with malaise and fever, generally a stronger antibiotic is necessary in order to cover a broad range of organisms that could possibly be present. Systemic symptoms may signify that the infection may be an uncommon invading organism or strep. If there is a possibility of a strep then, a rapid strep test is proposed if cost is of no concern. But if cost is a problem, treat with an antibiotic that covers for strep. It is crucial to treat and cure strep to prevent any underlining complications associated with strep such as a strep rash or scarlatina otherwise called sandpaper rash. A broader range antibiotic is needed such as an Extended Penicillin, Second Generation Macrolides or Fourth Generation Cephalosporin to inhibit for the possibility of strep throat infection.
For pregnant women, no antibiotic precautions are necessary as Penicillin. Macrolides and Cephalosporin and can be administered to pregnant women and children.
No necessary follow should be done especially if the symptoms improve but if symptoms persist or worsen, follow – up with a medical doctor is necessary almost immediately.
